You are hereby given formal notice of eviction from the Residence. This eviction is a result of the following actions:
- Unreasonable noise and activity at all hours of the night
- Destruction of property within Residence
- Remodel of Residence without Landlord's permission
- Failure to pay rent
- Behavior resultant in undue stress (vomiting, weight gain, change in feeding habits) to Landlord
- Refusal to comply with reasonable requests to make appearances
This notice is served with the mandatory 24 hours notice. If, at the end of the 24 hour period (ie. 0730 hours on 11/20/08), you are found to still inhabit Residence, you will be forcibly removed by Enforcement Official.
Any attempts to appeal this notice must be made in person to Landlord and Serving Authority at Mercy Gilbert Hospital, 3555 S. Val Vista Dr., Gilbert, AZ.
